2: Download The Free App
After you qualify and finish the enrollment process, you will download their free app and schedule your first telehealth visit with your Form Health doctor and Registered Dietitian. Before your first visit with your doctor, you will be asked to provide medical records and connect the Form Health team to your family doctor, or primary care provider (PCP). This will allow your Form Health doctor to have a full understanding of your medical background so they can identify your biologic blockers to weight loss and it will allow them to coordinate care as needed.
During your first visit, your doctor will conduct a comprehensive review of your medical history. You can expect to answer a lot of questions. This allows your team to develop a personalized weight loss pathplan for you. You’ll also have the opportunity to ask a lot of questions to understand your weight loss path and set realistic expectations.
Although you may be be prescribed an FDA-approved weight loss medication, your doctor will determine if it’s appropriate for you after reviewing your medical history.
3. Dietitian Meeting: Customized Diet and Coaching Plan
When you first meet with your dietitian, you’ll set achievable goals and receive guidance to overcome your unique challenges. You’re dietitian will help you develop a customized meal plan that addresses your needs and preferences with a focus on lean protein, fruits, healthy starches, and veggies. You’ll also be able to develop a plan to incorporate physical activity fitness regimen based on your personal abilities and interests.
4. Monthly Form Doctor Visits: Adjust Prescription Medication if Needed
You’ll have monthly Form Health doctor video visits to track your progress, discuss any concerns, and adjust medication. You’ll have video visits with your Registered Dietitian twice each month. This will help you continue to build on healthy habits and provide accountability. You can also message your clinical care team if you need them in between appointments. Who is Eligible for Form Health's Medical Weight Loss Program?
Form Health realizes weight loss begins with healthy habits and a lifestyle change. This is why their top priority is to help you adopt healthier eating habits, add physical activity to your routines, and make positive behavior changes. However, they also realize these approaches alone do not work for everyone. If these changes alone aren't enough, FDA-approved weight loss medications will be considered.
In order to qualify for a prescription medication through Form, you must:
- Age: You must be at least 18 years old
- BMI: You must have a BMI of 30 or higher or a BMI over 27 or higher with weight-related medical issues such as sleep apnea or high blood pressure
- Primary care provider: You must have an active family doctor or primary care provider (PCP) so Form Health can access your recent medical history and coordinate care with your doctor.
Additionally, if you suffer from certain medical conditions that put your safety at risk you may be deemed ineligible. This will be determined during your initial screen.

Form Health will only prescribe FDA-approved weight loss medications with strong scientific evidence to support their use. They will never recommend weight loss medications or supplements that are not approved by the FDA.

Form Health doesn’t have a specific length of time that patients should continue the program because each individual is different because some may show results quicker than others. There is no set end date.
